 Kids Heart Challenge
 Dear Crichfield Elementary Parent or Guardian,
It’s time for our traditional partnership event with the American Heart Association - Kids Heart Challenge – formerly Jump Rope for Heart.
Heart disease is the No.1 killer of all Americans and 80% of it may be preventable through lifestyle changes. Kids Heart Challenge teaches your child the importance of heart-healthy habits – a foundation for the rest of his/her life. This service-learning program will also share with your child the inspirational stories of children who have been touched by this disease while they raise funds to help kids like them - and save and improve all lives.
Raising money online really is the safest and easiest way to reach out to distant friends and family and even post to your social media accounts. Download the Kids Heart Challenge app or go to: http://www2.heart.org/goto/crichfield

This year we are only accepting online donations or e-checks via the Kids Heart Challenge App. We will not accept any cash or paper checks. Please remember you can see all of the Kids Heart Challenge characters come to life on the app as well!
During our Kids Heart Challenge program, your child will learn about taking care of their hearts and participate in cardio-pumping challenges. Plus, our school can earn PE equipment and your child can earn fun thank-you gifts – some that they will receive immediately and some that will arrive after our event.
We look forward to a successful and meaningful Kids Heart Challenge event at Crichfield this year!

- Registration forms
- Birth Certificate (must be 4 years old by August 1, 2021) only birth certificates issued by state or health department will be accepted.
- Proof of Residency (copy of a NIPSCO bill, TV Service bill, Trash bill; no general mail will be accepted)
- Current Immunization records
Important: all paperwork listed above must be turned in at time of registration to ensure your student’s admission.
No early paperwork will be accepted.
Students residing within the Crichfield boundaries will be given placement. Morning & Afternoon Classes.

If you are not the homeowner, please contact Mrs. Bye in the office for a Residency Affidavit. This form will need to be Notarized prior to registering and will serve as your proof of residency.

 ATTENTION PARENTS
New Arrival & Dismissal times effective for the 2020-2021 school year.
ARRIVAL
- 8:35 a.m. students allowed in to building; pick up breakfast (if needed) & go directly to classroom. NO EARLY ENTRY. STUDENTS MUST REMAIN IN THE CAR.
- 8:45 a.m. First bell rings
- 8:55 a.m. Second bell rings
- Students arriving after 8:55 must be brought to the office by a parent.
IN THE EVENT OF A 2-HOUR DELAY
- No breakfast is served
- 10:35 general admission
- 10:45 1st bell rings
- 10:55 2nd bell rings
DISMISSAL
All dismissal changes must be made by 2:30 p.m. No exceptions.
3:20 p.m.
Students who are not picked up in a timely fashion will be sent to Child Care. Fees will apply. |

- 2 boxes of Kleenex
- 3 tubs of Clorox wipes
- Toothbrush & Toothpaste
- Ziplock bags: 1 box snack size; 1 box quart size
- 3 glue sticks
- 3 tubs/refills baby wipes (no bleach/not antibacterial)
- 1 spiral notebook
- 2 or 3 complete changes of clothes.
Teacher will provide: pencils, crayons and scissors. Please do NOT send in: designer folders, loose-leaf paper, paints, markers, colored pencils, or other school supplies. The items listed above are all that is needed. |
 SLICER HEALTH CLINIC
 The Slicer Health Clinic, located at the High School, opens August 5th between the hours of 7:30 am - 4:00pm and offers confidential primary and urgent health care services. The Clinic can administer required immunizations for Kindergarten, 6th & 12th grade students. Sports physicals are also available for $20. Please call 324-1377 or email SHC@LPH.org for an appointment; walk-ins are accepted. The Clinic accepts all insurances and medical consents forms can be found on school websites. |
