Attached is a student enrollment form that ALL current students (grades 9 - 11) must complete for the 2016/2017 school year.  Each student will receive a copy of the enrollment form during school on May 4 but the form may also be printed at home.  All enrollment forms MUST be returned to school by Friday, May 13.
